We are seeking a Physician Assistant or Advanced Practice Nurse Practitioner to join our growing House Calls PCP practice in Mansfield-Arlington-Grand Prairie area. Applicants must live near the locations posted. Experience with Geriatric patients in the home environment required. Only qualified candidates will be considered.



Nurse Practitioner Responsibilities:

• Performing physical exams and patient observations for Geriatric patients in their home, ILF, ALF, Memory Care, Group Homes and Senior Communities.
• Recording patients' medical histories and symptoms in EMR system.
• Creating patient care plans and contributing to existing ones.
• Ordering, administering, and analyzing diagnostic tests.
• Identifying the need for and ordering medical equipment.
• Diagnosing and treating complex health issues.
• Managing, ordering, assessing effectiveness of medications and other treatments.
• Detecting changes in patients' health and modifying treatment plans, as needed.
• Consulting with healthcare professionals and families.
• Training patients and their families to manage and prevent illness and injury.
• MIPS/Quality Measures
• NO ON CALL.
• NO WEEKENDS.
• NO MAJOR HOLIDAYS.
• Health/Dental Ins, Retirement Account, Paid Time Off available to FT only.
• On-Site collaborating/supervising physician.
